Dysphagia in a Patient with Esophageal and Hypopharyngeal Cancers After Esophageal Reconstruction: A Pharyngeal Clearance Facilitating Maneuver | Dysphagia
A 59-year-old man who had squamous cell carcinoma of the esophagus with celiac nodal involvement (cT3N1M1, stage IV) underwent concurrent chemoradiotherapy followed by Ivor Lewis operation in 2006. Gradual onset of stricture of the esophagogastric anastomotic site was noted 10 years postoperatively, and he could not take food thicker than semiliquid. In 2018, gradual onset of odynophagia and hoarseness developed. The diagnosis of left posterior hypopharyngeal wall keratinizing squamous cell carcinoma (iT2N0M0, stage II) was confirmed. He underwent wide excision of left hypopharyngeal cancer and elective tracheotomy in April 2018. A neo-esophagus, which was made of a segment of transverse colon to connect the hypopharynx and jejunum, was also built to bypass the esophageal stricture. He also underwent adjuvant radiation therapy afterward. Dysphagia worsened after the operation with prominent post-swallow choking on liquids and even his own saliva. He was fed via a jejunostomy tube. A mo
